11/17/2023 EnchantaVaughn Approved QMHPA greeted Ms. Cosby upon arrival to the home setting. Upon arrival to the home setting, Ms. Cosby appeared to be in positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Cosby verbalized to QMHPA that she is glad that it is the weekend because she is going to go and relax herself. QMHPA informed Ms. Cosby that this session was going to focus on increasing her IADLâs. Ms. Cosby did not present any symptoms of SI or HI at the time of the session being conducted. QMHPA began the session by inquiring about Ms. Cosbyâs current thoughts, feelings and emotions. QMHPA discussed with Ms. Cosby being able to manage her money properly and budgeting techniques. QMHPA assisted Ms. Cosby with proper budgeting techniques and not spending her money in one day. QMHPA engaged Ms. Cosby in a skills building activity. QMHPA informed Ms. Cosby that the purpose of this activity was to assist Ms. Cosby with writing down things she need versus items she want. QMHPA explained to Ms. Cosby to be able to eliminate items she doesnât need to ensure she can afford the items she need. QMHPA reviewed Ms. Cosby ADL's to ensure she know what is expected from her every day. QMHPA engaged Ms. Cosby in another skills building activity. QMHPA informed Ms. Cosby that the purpose of this activity was to ensure that Ms. Cosby is aware of everyday living and how healthy and washing clothes on a consistent basis. QMHPA explained to Ms. Cosby about lack of motivation and becoming productive. QMHPA discussed with Ms. Cosby about independent living skills and quizzed her on what are some independent living skills. Towards the end of the session, QMHPA continued to educate Ms. Cosby on proper ADL's and independent living skills. Ms. Cosby appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Cosby explained to QMHPA at times when she is depressed or when she gets in that mood to where she doesnât want to do anything that is exactly what she is going to do. Ms. Cosby expressed to QMHPA that she does struggle with understanding what her needs are versus her wants. During the skills building activity, Ms. Cosby was able to provide QMHPA with feedback as it relates to her washing her clothes on a consistent basis and not allowing them to pile up. Ms. Cosby expressed to QMHPA that she feels that others in the home should take on the same task and complete house chores as well (she was referring to her boyfriend).
